Students: Complete this application form and download the All-State Application Signature Form (17k pdf) which must accompany your tape and $10/per audition registration fee. All tapes and fees must be packaged and sent by your school music director. Upon submission of this form you will receive an identification number to place on your audition tape with your Name, Instrument/Voice, School and City. Your ID number must also be indicated on the All-State Application Signature Form.
